Organize directory structure

Browse source 
- cardinal-plugin/ → plugins/ (matches Cardinal's plugin dir convention)
- index.html + game-state.json → site/
- OUTPUT_CATALOG.md → docs/
- Core Python files and tests stay flat (no import changes needed)
